Plan Details
- Individual membership plan: $96/year
- Family membership plan: $240/year
- The full membership plan fee is due upon joining the plan betwofore the reduced fee schedule can be applied toward your treatment.
- The membership plan is effective on the day which full membership payment is received and will expire on the day before anniversary date.
- Payments are due when services are rendered.
- It is the member responsibility to maximize their benefits by scheduling all appropriate appointments during the membership period.
- the member will not be refunded for unused benefits.
Plan coverage
- The members will receive one Dental Cleanings(Healthy gums), oral exam, oral cancer screening, unlimited digital X-Rays, and Fluoride treatment at no charge as benefits included in the discount plan.
- Other dental treatments including Oral surgery, Periodontal care, Rood canal, Crown, and Dental Implants will be covered at 40%.
- Orthodontics procedures will be covered at 35%.
